Support for You and Your Loved Ones
Whether you or your loved one has a chronic illness, is
living with a physical or developmental disability, or
is an older adult who needs more focused care, JFCS
Select Care is here to support you. A JFCS Select Care
professional can design a customized care plan,
coordinate services, provide referrals, and manage
multiple care providers to help you or your loved ones
maintain the best possible quality of life.
Experts with Strong Community Relationships
Our extremely qualified professional Care Managers have
degrees in nursing, social work, gerontology or related
fields, and have extensive knowledge of the availability
of specialized services in the community.
What a JFCS Care Manager Can Do for You
Care management begins with a comprehensive care
assessment completed by one of our experienced Care
Managers. The Care Manager will use the assessment to
outline short- and long-term needs, make recommendations
and develop a personalized plan of action.
Family assistance and legal and financial guidance are
part of what the Care Manager can provide.

nonprofit organization that has been dedicated to
providing quality social and behavioral health services
to the greater Tucson community for nearly 70 years.
